Bariatric Surgery has made massive weight loss commonplace. If you have ever watched My 600-lb Life you know what I am talking about. The show also airs in Colombia under a more appropriate title: Kilos Mortales. The title translates to Deadly Kilos, putting the emphasis on the fact that excess weight is deadly.
Tummy Tuck after weight loss before and after pictures – The pounds and inches are gone, but the skin that used to cover them persists. A Tummy Tuck (Abdominoplasty) tightens the abdominal muscles and removes the excess skin and fat that remain after massive weight loss.
Morbid Obesity is no joke. It is associated with multiple health problems including heart disease, high blood pressure, strokes, high cholesterol and diabetes. Moreover, it makes taking care of oneself challenging. An individual is considered morbidly obese if he or she is 100 pounds over their ideal body weight, has a BMI of 40 or more, or 35 or more and experiencing obesity-related health conditions, such as high blood pressure or diabetes.
Achieving Massive Weight Loss
Chest Reduction (Male Breast Reduction) before and after pictures – the chest can be left with excess skin too. In this case, gynecomastia reduction was combined with a skin reduction procedure to reveal a more masculine chest. The scar is in the inframammary folds under the breasts.
Before Weight Loss Surgery, losing weight depended solely on lifestyle modifications including diet and exercise. While effective, the process is slow and requires daily compliance. Today, weight loss surgery, also known as Bariatric Surgery, is an option that can significantly augment weight loss efforts. The availability of Bariatric Surgery has made massive weight loss attainable for more people.
Individuals meeting the criteria for surgical intervention have an option to help them meet their weight loss goals. Bariatric Surgery helps with weight loss in two ways. The first is mechanical and the second effects absorption. The mechanical aspect of weight loss surgery reduces intake. By making one feel full more quickly, negative feedback is provided for overindulgence. The result is that less food is consumed.
The second mechanism is decreasing the absorption of the food that is eaten. Internal bypasses are created surgically to shorten the gut and route the food past areas where it is better absorbed. By allowing food to pass through incompletely digested, the number of calories absorbed is reduced. Overall, less food is consumed and fewer calories are absorbed after Bariatric Surgery.
The Health Benefits Of Weight Loss
There are many Health Benefits associated with weight loss. Blood pressure can return to normal allowing the dose of antihypertensive medications to be decreased and sometimes even stopped. Elevated glucose levels can also decrease, which may mean stopping supplemental insulin and oral diabetes medications. As high blood pressure and diabetes come under control, the complications and mortality associated with them also decrease.
Where Did All This Skin Come From?
For many, the weight loss journey doesn’t end when the pounds disappear. When we gain weight our skin expands. The tension placed on the skin causes it to grow to cover the additional girth. Unfortunately, when we lose weight, the excess skin is not absorbed and often it does not recoil.
Mommy Makeover before and after photos – weight loss, like pregnancy, can lead to deflated breasts and loose bellies. The Mommy Makeover, originally designed to repair the damages of pregnancy, is modified for weight loss to provide enhancement of the breast and abdomen and create a more feminine physique.
The Excess Skin that remains after weight loss can be problematic and bothersome for Bariatric Surgery patients. Often, the residual skin becomes lax and folds upon itself causing pain, rashes and infections. Additionally, excess skin makes it challenging to find comfortable clothing and difficult to perform many activities of daily living.
Massive Tummy Tuck before and after pictures – This tummy tuck has been “super-sized” to address over stretching of the abdominal muscles and generous amounts of excess, redundant skin.
Excess skin of the arms, neck, chest, abdomen, back and thighs is common after massive weight loss and the reason many Bariatric Patients seek Post Bariatric Plastic Surgery to complete their transformation.
Post Bariatric Plastic Surgery
Arm lift before and after pictures – The weight is gone, but the arms still don’t fit in a sleeve. An Arm Lift (Brachioplasty) removes the excess skin and slims the arms allowing them to better fit into a sleeve.
Post Bariatric Plastic Surgery is a specialized branch of Plastic Surgery that deals with the extra skin left after weight loss. It is often referred to as skin removal surgery. Many of the procedures are similar to those used for normal amounts of weight loss or after pregnancy, but often they are modified (super-sized) to treat the enormous skin excesses seen after massive weight loss.

Gynecomastia Reduction is performed for male breast enlargement. These before and after pictures show a reduction in breast projection on the oblique view.
Gynecomastia is the growth of excess breast tissue in the male chest. The resulting mass causes fullness, embarrassment and often tenderness, for men who develop what is colloquially known as “man boobs.” Gynecomastia Reduction removes the excess breast tissue and restores the masculine physique. This Gynecomastia Reduction Video illustrates and narrates several typical cases.
The Gynecomastia Reduction Before and After Pictures shown above and below are of the same patient. The oblique views (above) show the reduction in projection that Male Breast Reduction can achieve. The front view (below) reveals that more tissue was removed from the larger left side. The Gynecomastia Reduction improved his breast symmetry by equalizing his breast volume, reducing the size of his areolae and elevating the position of the left nipple. Areola Reduction and Nipple Reduction are separate procedures used to reduce the size of the areolae and nipples. In this case, these ancillary procedures were not necessary.
Gynecomastia Reduction is performed for male breast enlargement. These before and after pictures show improved symmetry and reduction in breast and areolar size.
Gynecomastia Reduction Video
The following Gynecomastia Reduction Video reviews several typical Male Breast Reduction Surgeries, and narrates the indications, procedure and recovery. This video originally aired on the San Francisco Bay Area’s News Station, KRON 4.
Gynecomastia Reduction for Tubular Breasts
Tubular Breasts can occur in both men and women. For women, the goal of Tubular Breast Surgery is the creation of normally shaped and symmetrical breasts, For men, the goal is the same as with all Gynecomastia Reduction Surgery: the reduction and flattening of the chest.
These before and after photos show how Gynecomastia Reduction can improve the appearance of Tubular Breasts for men.
Gynecomastia Reduction for Bodybuilders
Gynecomastia Reduction is particularly useful for the reduction of breast buds in bodybuilders. As they lean out for competition, the fat flattens; however, the gland does not. This causes increased projection of the nipple-areolar complex. These buds are distracting, and they lead to penalties during competition. Since Gynecomastia can occur with steroid use, removing the mass removes the stigmata.
These Gynecomastia Reduction before and after photos show reduction of the glandular tissue under the nipples for a Bodybuilder.
Gynecomastia Reduction for Asymmetry
Most breasts are asymmetrical. For men this is usually subtle, but when Gynecomastia forms, the asymmetry can become quite pronounced. In the case below, the left breast is significantly larger than the right. Even in a dress shirt, the left breast protruded and was a source of embarrassment for my patient. Gynecomastia Reduction was performed on both sides, and more tissue was removed from the left to provide a flattened, masculine chest.
Sometimes Gynecomastia occurs primarily on one side of the male chest. These before and after photos show the improvement of breast symmetry after Gynecomastia Reduction.

A Chin Implant increases projection, provides enhanced facial balance and harmony, and is the most common method of Chin Augmentation.
Chin Augmentation balances the lower face when the chin is small or weak. The most common technique for Chin Augmentation utilizes a Chin Implant placed on the front surface of the mandible (the lower jaw bone). The Chin Implant supports the soft tissue of the chin, pushing it forward. The result is improved facial harmony by balancing the upper and lower face and better defining the transition between the face and neck.
Chin Augmentation with Chin Implant Video
Chin Augmentation is most commonly used in cases where the front of the chin is located behind the front of the lips when viewed on profile. The following Chin Augmentation Video Presentation shows several examples of Chin Augmentation using Chin Implants. This is the most common procedure used to augment the chin profile, and balance the lower face when the chin is small. This Plastic Surgery Video Presentation is a segment taken from my TV show Body Beautiful, which airs on the San Francisco Bay Area’s News Station, KRON 4.
Chin Augmentation with a Chin Implant is an effective, cosmetic surgery, which can dramatically change your facial profile. Computer imaging can facilitate communication between you and your surgeon as many options exist in the types, sizes and shapes of Chin Implants. Additionally, computer imaging can help visualize the aesthetic nature of the results. Like any surgical procedure, there are risks and benefits which should be discussed with your Board Certified Plastic Surgeon prior to surgery.
Chin Augmentation before and after pictures: Chin Implants can increase chin projection and definition. Before Chin Augmentation, her chin appears flat, is poorly defined and allows the lip and neck to blend into each other. After Chin Augmentation, the Chin Implant provides a defined chin, which clearly separates the lip from the neck. Additionally, the chin’s projection now matches the lips’ and balances the lower face.
Chin Augmentation vs. Jaw Surgery
Chin Augmentation with a Chin Implant is not jaw surgery. It enhances the apparent size of the jaw, but it does not change the mandible or the way the teeth come together. The implant is applied to the front surface of the bone to provide a cosmetic result. Jaw surgery, also known as Orthognathic Surgery, involves cutting the actual jaw bone.
Procedures like the Sliding Genioplasty, cut the mandible and slide the chin piece forward. Metal plates and screws are then used to hold the front of the mandible in the desired position. Since just the front of the mandible is advanced, there is no change in how the teeth come together with this surgery.
When malocclusion exists, the teeth do not come together properly. The most common treatment for this is orthodontia. While orthodontia can help one’s bite, it moves the teeth in the jaw, rather than move the jaw and the teeth. In severe cases of malocclusion, jaw surgery may be recommended.
Operations exist to move the upper and lower jaw. One such operation is the Sagittal Split Osteotomy. This corrects retrusion of the mandible, better known as a severe overbite, by moving the entire lower jaw forward. The main goal is to align the teeth, but it has the added benefit of adding projection to the lower jaw and helping with facial harmony. This is a specialized procedure, only performed for specific indications, by specially trained orthognathic surgeons, usually an oral and maxillofacial surgeon in conjunction with an orthodontist.
Cosmetic Chin Surgery
Chin Implants provide stable Chin Augmentation. In the above Chin Implant before and after pictures the patient is smiling. Even with animation, the enhanced projection is maintained and the definition of the neck line is clearly defined. In cases where there is disproportionate fat of the neck, Chin Augmentation can be combined with Neck Liposuction to further enhance your profile.
When dental occlusion is good, or has been corrected with orthodontia, and the concern is primarily the outward appearance of the chin, jaw surgery is usually not needed. This is where Chin Implants have a niche. At your consultation appointment, the best size, shape and type of chin implant is determined.
Chin augmentation surgery is performed at an accredited outpatient surgery center. During the procedure, an incision is made either inside the lower lip or under the chin for access. A pocket is made and the implant placed in front of the mandible to provide additional projection. Recovery is variable, depending on the type and size of implant, but is normally a few weeks.

Today’s episode of Body Beautiful discusses Eyelid Lifts including: the upper eyelid fold and skin excess and lower eyelid bags and loss of tone. Blepharoplasty can correct multiple problems with the eyelids and encompasses many specific techniques. The following Blepharoplasty Video illustrates several of these procedures and demonstrates what is possible with carefully planned Eyelid Rejuvenation Surgery.
Upper and Lower Blepharoplasty Before and After Pictures: Sagging upper eyelids, caused by excess upper eyelid skin, leads to a tired and disinterested appearance. Blepharoplasty can open your eyes to a more youthful, bright and alert appearance.
The Eyelid Lift Video segment below is from the Bay Area’s News Station, KRON 4’s, Body Beautiful. The show features the latest in Cosmetic Plastic Surgery News, and uses before and after pictures to show what is possible for actual patients.
Eyelid Lifts and Double Eyelid Surgery Video Presentation
Double Eyelid Surgery
Asian Eyelids are anatomically distinct from Caucasian eyelids. Asian eyelids often lack a distinct palpebral fold, and when the fold does exist, it may be indistinct or crease at multiple levels. An example of this is show below. The patient’s right upper eyelid has multiple folds. She has a high fold, a low fold and a third fold in between on the inner eyelid. Her left upper eyelid has a single fold, but it is low and becomes indistinct as it extends toward the nose medially.
Double Eyelid Surgery can correct low, indistinct or multiple eyelid folds. These double eyelid before and after pictures show how the folds can be raised, sharpened and made more symmetrical.
Double Eyelid Surgery is the procedure used to raise the eyelid folds, reduce multiple folds and improve the symmetry of the upper eyelids. The after picture reveals improvement in all these areas. Trying to form a fold when there is none, or it is in the wrong position, is more difficult than just removing excess skin, so revisions are more common with Double Eyelid Surgery when compared to standard Upper Blepharoplasty.
Eyelid Surgery Consultations
Eyelid Surgery is one of the most meticulous of all cosmetic plastic surgery procedures. While the operation can be performed safely and reproducibly, even under local anesthesia when desired, you can improve the chances of a good result by seeking an experienced and Board Certified Plastic Surgeon or Oculoplastic Surgeon. A plastic surgeon can also address other aspects of Facial Plastic Surgery and maintain facial harmony with other rejuvenating procedures such as: Brow Lifts, Face Lifts or Neck Lifts. This is particularly important if these areas are aging at the same rate as the eyelids.
To schedule a personal, private, in-person consultation call (925) 943-6353, today. The goals, procedure and anticipated recovery, specific to your needs, will be reviewed in detail.
The Tummy Tuck is half of a Mommy Makeover. The patient shown above wanted her pre-baby belly and breasts back. The combination of Abdominoplasty and Breast Augmentation with a Breast Lift restored what nature had given and taken away.
The Tummy Tuck remains one of the most sought after cosmetic plastic surgery procedures. Also termed, Abdominoplasty, it literally means belly changing, and that is exactly what it does.
Tummy Tuck and Mommy Makeover Video
Today’s episode of Body Beautiful reviews the Tummy Tuck procedure, and beautifully illustrates what Abdominoplasty can do to restore abdominal contours often lost after pregnancy or changes in weight. This video segment is from the Bay Area’s News Station, KRON 4’s, Body Beautiful. The show features the latest in Cosmetic Plastic Surgery, and uses before and after pictures liberally to demonstrate what is possible.
Tummy Tuck After Pregnancy or Weight Loss
After pregnancy or weight loss, the abdomen is often left flaccid. The muscles, even when strong, are stretched out and cannot support the abdominal contents the way they used to. The skin itself is stretched out, often leaving stretch marks and rolls. The result: clothes don’t fit the way they used to.
What is a Mommy Makeover?
The breasts often suffer a similar fate, so restoring breast volume and perkiness is often performed at the same time as the Tummy Tuck via Breast Augmentation, Breast Lift or the combination Breast Augmentation Lift.
More Mommy Makeover before and afters: Again, Abdominoplasty is combined with Breast Enhancement. In this case, a Breast Lift was combined with a Tummy Tuck for a uplifted, aesthetic result.
Because this is commonly a problem after pregnancy, the combination of Breast Enhancement and Tummy Tuck goes by the name Mommy Makeover. In the Mommy Makeover Before and After Pictures above, a Breast Lift (Mastopexy) was combined with a Tummy Tuck for maximum aesthetic results.
Tummy Tuck or Mommy Makeover Consultations
There is no substitute for proper training and experience. When considering a Tummy Tuck, Mommy Makeover or any Cosmetic Plastic Surgery, consult with an experienced and Board Certified Plastic Surgeon. If you are in the San Francisco Bay Area, call (925) 943-6353 to schedule a private and personalized consultation appointment, today.
The Brazilian Buttocks Lift (BBL) is a specialized procedure designed specifically to enhance both the size and shape of the buttocks. During the procedure, Fat Grafting is used, so you need to have excess fat elsewhere on your body to be a candidate for the BBL. San Francisco Bay Area Plastic Surgery patients have been asking for this procedure with greater frequency every year.
In the above BLL patient, fat was removed from the lower back, flanks and thighs and was injected into the buttocks.
As a specialist in Cosmetic Plastic Surgery including both Facial Plastic Surgery and Body Work such as: Tummy Tuck, Breast Augmentation, the Brazilian Butt Lift has become the go-to procedure for Buttocks Enhancement.
Fat Grafting to the Buttocks Video
The follow video segment from KRON 4’s Body Beautiful shows BBL before and after pictures, and we review many of the details of Fat Grafting to the Buttocks.
This following video is not a substitute for an in-person consultation with an experienced and qualified plastic surgeon. If you would like to schedule an appointment with Dr. Mele call (925) 943-6353 today.
Brazilian Butt Lift Video
Brazilian Buttocks Lift Safety
The Brazilian Buttocks Lift is a surgical procedure. The most important aspect of planning for any elective procedure is your safety. For this reason I perform BBL only in accredited outpatient surgery centers, with the assistance of an Anesthesiologist. Their job is to keep you comfortable and safe, allowing me to concentrate on the aesthetics.
The American Society for Aesthetic Plastic Surgery (ASAPS) and The American Society of Plastic Surgeons (ASPS) have both recently issued guidelines for BBL safety. Both professional societies recommend that fat be injected only between the skin and the muscle. Major complications, and even deaths, have been reported with fat injections into the muscle and associated injury to the large veins deep inside the buttocks.
No deaths have been associated with superficial fat injections. In this location, the fat gives more enhancement anyway, so it’s a win-win — more effective and safer. Be certain to ask your surgeon at what level they inject fat, and if they are injecting into or deep to the muscle, find another surgeon. Fat grafting is less effective and more dangerous in these deeper layers.

Summer may be over, but Breast Augmentation is still one of the most frequently requested Cosmetic Plastic Surgery procedures here in the San Francisco Bay Area. For many, this is the best time of year for the procedure. As the weather cools down, it is easier to cover up during recovery. Moreover, you will be well healed by the time Spring comes around.
Gummy Bear Breast Implants are the newest technology. With a strong, cohesive gel inside, they can provide more projection, less rippling and the safety reduced gel migration.
This Breast Implant Video Presentation illustrates the options and the results of Breast Enlargement. There are many reasons for Breast Augmentation. Sometimes it’s for size. Sometimes it’s for shape. The latest breast implants provide shape and support for the breasts with the increased safety of strong, highly cohesive gel inside them. These “Gummy Bear” Breast Implants reduces gel migration and some other problems seen in the past after Breast Augmentation with Silicone Gel Breast Implants.

Otoplasty (cosmetic ear surgery) is performed for men and women, children and adults. The most common reason is to reduce the appearance of prominent ears, sometimes called ear pinning.
Otoplasty – Ear Pinning
In the San Francisco Bay Area, Otoplasty is a popular procedure. This cosmetic ear surgery is used to reduce the appearance of Prominent Ears, and it is probably the most commonly performed cosmetic plastic surgery for children.
The video segment below is from the San Francisco Bay Area’s News Station, KRON 4’s, Body Beautiful show. Included are several examples of Otoplasties and many of the details of cosmetic ear surgery are explained.
Cosmetic Ear Surgery Video (Otoplasty)
Short Hair and Prominent Ears
Short hair does not disguise prominent ears. However, since the incisions are hidden in the fold behind the ears, short hair is not a problem for concealing the Otoplasty incision.
Many men and women leave their hair long to hide their prominent ears, but short hair is not an obstacle for Otoplasty. The incision for cosmetic ear surgery is hidden in the fold behind the ear, and usually heals very well. It is not necessary to cut your hair before Otoplasty; however, after the procedure, you’ll have the option to do so.
To schedule a personal consultation appointment for yourself or your child, call (925) 943-6353, today. Whether it’s just one ear or both, ears that are too prominent or folded over, Otoplasty can help. To learn more about cosmetic ear surgery, visit the links embedded in this post; however, only an in-person consultation can be tailored to your specific needs.
Massive weight loss will shrink our volume, but does not reliably shrink our skin. In the before and after pictures above, breast volume is restored with breast implants. Sagging breast skin is tightened with a breast lift. Excess abdominal skin is removed, and the belly bulge caused by stretched out abdominal muscles, is flattened by tightening the muscle sheath.
Post-Bariatric Plastic Surgery is Plastic Surgery after Massive Weight Loss. Whether this is after a lifestyle change or after Bariatric Surgery, the amount of skin that persists after weight loss is often large and frustrating. This Post-Weight Loss Plastic Surgery Video reviews several of the most common treatments including: Lower Body Lifts, Tummy Tucks, Breast Augmentation, Breast Lifts and Arm Lifts. The results of Post-Bariatric Plastic Surgery are illustrated and narrated in this video with before and after pictures, which describe and demonstrate what Board Certified Plastic Surgeons can do for the skin excess that often occurs after weight loss.
Post-Bariatric Plastic Surgery Video
Extremity Tightening Procedures
The majority of skin tightening procedures performed after weight loss are for the trunk; however, the face, arms and legs can also develop excess skin. Face Lifts (Rhytidectomy), Arm Lifts (Brachioplasty) and Thigh Lifts can help with excess skin on the face/neck, upper arm and thighs. Below is an example of an arm lift.
Brachioplasty literally means arm changing. An arm lift can change the way an arm looks, feels and fits into sleeves.
Brachioplasty changes the arm by removing the excess skin that often hangs off the upper arm after weight loss. The skin is removed from the lower border of the upper arm, leaving a scar along the bottom of the arm. This is the only procedure that can remove large amounts of excess skin.
Post Bariatric Plastic Surgery Consultations
As an active member of the American Society of Bariatric Plastic Surgeons, post bariatric plastic surgery is an area in which I specialize.
The American Society of Bariatric Plastic Surgeons was formed by Board Certified Plastic Surgeons specializing in skin tightening after massive weight loss.
When searching for an experienced post-weight loss plastic surgeon, be certain to look for a Board Certified Plastic Surgeon who is a Member of the American Society of Plastic Surgeons. Additionally, members of the American Society for Aesthetic Plastic Surgery specialize in the cosmetic side of plastic surgery, and members of the American Society of Bariatric Plastic Surgeons demonstrate additional dedication to the cosmetic side of post-bariatric plastic surgery.

Male Breast Reduction is one of the most requested cosmetic plastic surgery procedures, year after year.
Male Breast Reduction Video (Gynecomastia Reduction)
Gynecomastia is the medical term for Male Breast Enlargement. The enlargement can be isolated to the nipple areolar complex (puffy nipples), or may involve the entire chest. This Gynecomastia Reduction Video reviews the most common treatments and illustrates the results of Male Breast Reduction with multiple, narrated male breast reduction before and after pictures.
How Is Gynecomastia Reduction Performed?
Gynecomastia Reduction is often a combination of procedures. The most common combination is Liposuction plus direct excision of any firm glandular tissue which remains attached to the back of the nipple.
In situations where the skin is loose or the gland is saggy, skin reduction may also be necessary. The key it to pick the procedure, or combination, that gets the job done.
With good skin tone, even large breast reductions can be performed through small, minimally invasive incisions.
