Every year the American Society of Plastic Surgeons (ASPS) and the American Society for Aesthetic Plastic Surgery (ASAPS) publish their Cosmetic Plastic Surgery Statistics. The 2011 statistics show a gradual increase in the number of cosmetic plastic surgery procedures.
Cosmetic Procedures on the Rise
Overall Americans had 13.8 million cosmetic procedures in 2011. The majority of these were minimally invasive procedures with the neuromodulators (Botox, Dysport) and dermal fillers (Restylane, Juvederm, Perlane and Radiesse) accounting for the majority of treatments.

So what is the latest in Rhinoplasty Walnut Creek? San Francisco Bay Area plastic surgery patients want to know. I explore the topic with an eye on history because new nasal surgery techniques come and go, but the truly great ideas persist. The basics of the “modern” rhinoplasty have been around for centuries; however, the advent of safe and effective anesthesia has allowed us the operative time to work on the fine details with high patient satisfaction.
Closed Rhinoplasty vs. Open Rhinoplasty
The debate over which technique is better, closed rhinoplasty or open rhinoplasty, continues to this day. Each provides some advantages and disadvantages, and the final determination of which technique is best for your nose will depend on an careful, professional evaluation by a Board Certified Plastic Surgeon or Board Certified Otolaryngologist (also called Facial Plastic Surgeons though the ABMS does not recognizes this as a separate Board Certification).
Closed Rhinoplasty
Prior to the 1900′s, most rhinoplasty was performed “closed”. Pioneers like Joseph, Freer, Killian and Cottle showed us what can be done with the closed rhinoplasty technique, and many of the instruments used for septoplasty today, still bear their names.
The closed rhinoplasty avoids the small incision between the nostrils necessary for the open technique, and closed nasal surgery works well for dorsal reductions and narrowing of the boney and cartilaginous nasal sidewalls. For these standard rhinoplasty modifications, the open technique does not provide much advantage.
Since the closed technique incisions are all inside the nose, they are hidden. However, the small external scar needed in the open technique usually heals extremely well and is very difficult to see in most circumstances. The closed technique may have less post-operative tip swelling, but when similar operations are performed, it is difficult to demonstrate the difference.
Open Rhinoplasty
Open rhinoplasty was advanced by Rethi, Padovan and Goodman for the tip rhinoplasty, and was advocated for its superior visualization of the nasal anatomy.
The difference between open nasal surgery and closed nasal surgery is the 1/4 inch incision made between the nostrils. This small incision allows the nasal skin to be lifted, like the hood of a car. While the nasal tip cartilage can be seen with the closed approach, it is necessary to deliver one side at a time for tip work. The open technique reveals both sides simultaneously, and facilitates the symmetry of the result.
Nasal Autografting
Another leap forward in the advancement of the 21st century rhinoplasty was the use of autografting. Cottle, Skoog and more recently Sheen, described combining resection of the nasal dorsum (the bump on the nose) with augmentation to achieve an aesthetic nose. The technique involves removing cartilage from where there is an excess, and adding it to areas of deficiency. Sometimes the amount removed is insufficient, and additional cartilage is borrowed from the nasal septum, ear and less commonly the rib.

Gaspare Tagliacozzi - the Father of "Modern" Plastic Surgery
Rhinoplasty (nose jobs) has continued to evolve since the early descriptions by Sushruta in 1000 to 600 BC. A giant leap forward was made in the 16th century by Italian surgeon Gaspare Tagliacozzi. Tagliacozzi championed the Italian Method of Plastic Surgery, and made nasal surgery more practical. His ideology is documented in De Curtorum Chirurgia per Insitionem Libri Duo, published in 1597. Professor Tagliacozzi also provided us with one of my favorite plastic surgery quotes:
“We restore, rebuild, and make whole those parts which nature hath given, but which fortune has taken away. Not so much that it may delight the eye, but that it might buoy up the spirit, and help the mind of the afflicted.”
Modern Rhinoplasty
New methods for reliably reducing and augmenting the nose make the procedure more versatile, safer and more predictable. Computer imaging helps to relay the desired goals between you and your rhinoplasty surgeon, and the future looks promising. Tissue engineering will one day allow for the tissues of the nose to be designed and available off-the-shelf. This will be extremely helpful for reconstructive nasal surgery, when either due to cancer, trauma or dysphoria.
Nasal surgery is one of the most personalized and one of the most demanding of all the cosmetic plastic procedures in which I specialize. Rhinoplasty is one of the few surgeries where a millimeter of change can make a big difference.

Sushruta, the Original Nose Job and Ancient Plastic Surgery
Sushruta - The Father of Indian Surgery and Plastic Surgery
While many consider plastic surgery a relatively new discipline, its origins are documented more than 4000 years ago by the Indus River Civilization of India. In Sushruta’s Compendium (around 1000 BC – 600 BC), Sushruta documents ancient medicine, surgery and midwifery 150 years before Hippocrates.
The Nose Job is one of procedures which defines early Plastic Surgery, and Sushruta is credited as the first to describe the details of rhinoplasty. His compendium shows clearly that surgery and cosmetic plastic surgery have always been linked. Sushruta’s techniques spread from India to Persia, Arabia, Egypt and eventually Europe, and some methods are still used today.

With the advent of computer imaging, communicating what is possible has become a little easier. And it works both ways, in that it also makes it easier to figure out what my patients are asking me to do.
There is another benefit of computer simulation, a reduced need for revision surgery caused by miscommunication. Undo is a simple command on the computer, and it allows for a no risk exploration of all the possibilities. This way, when it is time for your nose job, the plan has been clearly communicated. Revision surgery is not unusual for rhinoplasty; however, it is more likely to be for fine tuning rather than for completely redoing when the plan is clear from the onset.

The previous two posts were about Rhinoplasty and Chin Augmentation. Sometimes combining procedures gives the best result. The Mommy Makeover, for example, combines breast enhancement with a tummy tuck. The two areas are anatomically independent, but by addressing two areas simultaneously, the recovery periods can be combined, and the results appreciated more quickly. When the two procedures are anatomically related, like when Rhinoplasty and Chin Augmentation are combined, simultaneous treatment allows for the creation of the optimal facial proportions and a pleasing cosmetic result.

The Balanced Nose and Chin
The woman above presented with complaints of a bump on her nose (a dorsal hump) and a weak nasal tip that curved downward. She also had a small chin (microgenia) with a chin prominence well behind the projection of her lips. A rhinoplasty and chin augmentation were performed to smooth the dorsal hump, define and elevate the nasal tip and bring the chin forward. By combining the procedures, not only could each complaint be addressed, but the mid face and lower face profiles could be made harmonious.
How the Nose and Chin are Related
A small chin makes the nose appear larger, and visa versa. Modifying one will have the opposite effect on the other. In other words, if you make the chin larger, the nose will appear relatively smaller. By combining the procedures, the aesthetic balance of the nose and chin can be addressed, while each individual area is enhanced.
How to Choose a Plastic Surgeon for Combination Procedures
Combination procedures are best performed by a Board Certified Plastic Surgeon who is experienced with each component, and with the synergy that the combination provides. If you are looking to combine surgeries, you want a Board Certified Plastic Surgeon with experience in a wide range of cosmetic plastic surgery procedures, and the ability to seamlessly combine them.
How to Check if Your Plastic Surgeon is Board Certified
Rhinoplasty, nasal surgery, or if you prefer, the nose job, is one of the top five cosmetic plastic surgery procedures. In 2009, rhinoplasty was the fourth most popular cosmetic plastic surgery procedure, and the second most popular for men. Overall, last year, 138,258 rhinoplasty procedures were performed.
Rhinoplasty was performed to remove the dorsal hump and refine a boxy tip.
With the advent of computer imaging, the ability to communicate what can (and can’t) be done to the nose has been greatly enhanced. Polaroid film and hand sketches have been replaced with digital images that can be modified directly on the computer screen. While limitations remain, computer imaging transforms the consultation, and the flow of information, in both directions.
Computer imaging enhances my ability to both glean what my patients are seeking, and relay what I am hearing back visually, as a picture. While computer simulation cannot be a guarantee of results, improved explanations, and resulting expectations, make the process and outcome more predictable. Since it is your picture, it is easier to see what changes will look like on your face, rather than looking at someone else’s before and after pictures.
One of the most common nasal deformities (for lack of a better word) that I fix is the dorsal hump. A dorsal hump is the bump on the nose, best seen in profile. It is often a part of ones ethnicity, and how much modification is desired becomes critical. Computer imaging allows us to try out several approaches:
1) Reducing the bump
2) Removing the bump
3) Overresecting the bump
While these words may give you an idea of what the approaches are, modified pictures showing your nose with a smaller bump, a straight nasal profile or a slight scoop, makes the planned outcome much more tangible.
Computer imaging works well for tip modification also. The width, height and projection of the nasal tip can be modified on the screen. By knowing your options, you can be confident that the best decisions are being made.
For the patient show above, both areas (the nasal dorsum and tip) were modified. She had a dorsal hump and a wide boxy tip that she wanted refined. Both changes were demonstrated, before surgery, during her initial consultation appointment. This allowed me to determine the range of modification desired in both areas. The changes made served as my guide during surgery.
Since the surgery has to be completed with a scalpel (and not photoshop) there are some limitations to what can be done physically, but computer imaging for nasal surgery is a great tool that really helps me make my patients happy. It adds predictability to a surgery where small changes go a long way. The American Society for Aesthetic Plastic Surgery (ASAPS) and the American Society of Plastic Surgeons (ASPS) have released their annual statistics on cosmetic plastic surgery for 2009. Surgical and non-surgical procedures are tallied and compared to previous years. Overall the numbers are down 2% from 2008, but still up 147% since 1997, when the statistics were first compiled. Of the almost ten million procedures performed in 2009, 85% were non-surgical and 15% were surgical. He are some quick facts, a link to the full statistics can be found at the end of this article.
