The Tummy Tuck remains one of my most frequently performed cosmetic plastic surgery procedures, and one of my favorites. Plastic Surgeons have to be a bit obsessive to get the very best results. The Tummy Tuck, or Abdominoplasty, gives us the ability to not only remove fat, like Liposuction, and remove extra skin like a panniculectomy, but also allows a Plastic Surgeon to tighten the abdominal wall muscles. Tightening the abdominal muscles is the only way to tighten the foundation of the abdominal wall and obtain the flattest possible results.
Muscle Tightening – The Foundation of the Tummy Tuck
The Tummy Tuck Before and After Pictures above show what can be accomplished. The bulge that starts well above her belly button is from laxity of the muscles that make up the abdominal wall. This is often seen after pregnancy, and no matter how many sit-ups are performed, if the muscle fascia is over-stretched, the muscles will not tighten properly.
During your Abdominoplasty, the tough case the covers the front of abdominal muscles (the fascia) is tightened. Sutures are placed that act like an internal corset and flatten the belly. It is effective as long as the fascia is normal and the belly is not full of fat. Tummy Tucks are best performed when we are near our ideal body weight and are not planning to have more children.

Abdominoplasty Before and After Pictures: The front view of the above Abdominoplasty shows how much the abdominal muscles and skin are tightened. The shadows of the muscles are brought closer to the midline and skin is much smoother. The belly button is also rejuvenated.
Skin Tightening – The Icing of the Tummy Tuck
Sometimes the abdominal muscles are in good shape, but there is too much skin covering them. Abdominoplasties directly remove extra skin right off the front of the belly, and the skin removal is sometimes continued around the sides for an extended Tummy Tuck, if the skin excess continues around to the sides. An Abdominoplasty will not tighten the skin of the back. This is why the Lower Body Lift was invented.
Fat Removal for Disproportionate Fat
Liposuction is frequently combined with the Tummy Tuck to maintain the trunk’s proportions. If the sides or back are full, but the skin is tight, Liposuction can be used to reduce this fullness. More recently, this fat has also been repurposed to enhance the buttocks as part of a Brazilian Butt Lift (BBL). In the BBL, fat is grafted into the buttocks to enhance its shape and restore absent volume. An example is shown below.
Tummy Tuck and Brazilian Butt Lift Before and After Pictures: In this case, Abdominoplasty is combined with Liposuction of the back and flanks. The fat is harvested and grafted into the buttocks to create fuller, curvier buttocks.

When is a Mini Tummy Tuck enough? It’s a question I answer every day. Cosmetic Plastic Surgery options for Abdominal Rejuvenation range from Liposuction to Tummy Tuck, with the Mini Tummy Tuck covering some of the middle ground. Unlike Liposuction alone, the Mini Abdominoplasty can tighten the skin and muscle wall of the lower abdomen. But a Mini Tummy Tuck doesn’t do everything a Tummy Tuck can, and it’s important to know the benefits, as well as the limitations, before proceeding.

What Is A Foopa?
Foopa comes from the acronym F.U.P.A., which is short for Fat Upper Pubic Area. Originally, this was meant to identify a disproportionate collection of fat above the privates and below the belt line; however, its meaning has expanded to include all lower belly fat, especially the fat that sticks out above a C-section scar. Foopas cause the fullness seen in “Mom Jeans.” Larger foopas can simulate the appearance of early pregnancy.
Can A Mini Abdominoplasty Fix A Foopa?
Small foopas, both above and below the belt-line, can often be fixed with a Mini Abdominoplasty. Larger foopas are associated with larger amounts of abdominal laxity, and often require a Full Tummy Tuck for the best results.
Mini Tummy Tuck Quick Check
Here is an easy way to check if a Mini Tummy Tuck is enough. Cover the area below the belly button and look at the belly. If the area above the belly button looks good, a Mini Tummy Tuck will often be enough to obtain a good result. If there is loose skin above the belly button, or if the abdomen bulges above the belly button, a Full Tummy Tuck will give a superior result. If the area below the belly button is full, but the skin is tight, Liposuction may be the easiest fix.

Mommy Makeovers are a combination of cosmetic plastic surgery procedures that enhance, or repair, the breasts and belly. Since both areas take a hit with pregnancy, it is most commonly performed for mommies, but Mommy Makeover procedures are also performed after weight loss. Post Bariatric Surgery patients often have loose skin on their breasts, bellies and other areas, so Mommy Makeovers are performed after weight loss also.
The Mommy Makeover helps by enhancing both the breasts and the belly in a single combination plastic surgery procedure. Above, a Breast Augmentation Lift (Augmentation Mastopexy) is combined with a Tummy Tuck for the optimal result.
The Mommy Makeover is a not a specific operation, but rather a combination of breast and belly enhancement procedures. Each area is evaluated separately and we choose one from column A (breast enhancement) and one from column B (belly enhancement) and the combo plate is termed a Mommy Makeover. The ability to mix and match is what makes the procedure so powerful. The best procedures for each area are combined.
Mommy Makeover – Breast Enhancement
Breast Enhancement is half of the Mommy Makeover. In this case, a Breast Augmentation Mastopexy is performed. A Breast Implant is used to increase the breast volume, while a Breast Lift is used to raise the nipple and reshape the breast. A larger implant without a lift would not make a pretty breast. A larger implant would make larger breasts, too large for this patient’s desires. Additionally, the larger implant would push the nipple lower on the breast.
Breast Enhancement includes many options. The most common are Breast Augmentation and Breast Lifts, though for those women whose breasts do not shrink after pregnancy, Breast Reduction is also a possibility. Below is a list of Breast Enhancement procedures with links to additional information:
The gold standard of Belly Enhancement is the Tummy Tuck. It reduces loose abdominal skin, removes excess belly fat and tightens muscle laxity including rectus diastasis (muscle separation).
Belly Enhancement also includes several options. The classic Mommy Makeover is a Breast Augmentation combined with a Tummy Tuck. The Tummy Tuck (Abdominoplasty) is the work horse. Abdominoplasty can correct loose skin, lax muscle fascia and remove excess fat. It is the only procedure that addresses all three areas over the entire belly. Sometimes, a Tummy Tuck is more than is needed so “smaller” procedures, like Mini Tummy Tucks or Even Liposuction can be substituted for selected patients. Liposuction can also be added to help with other problem areas like the love handles, muffin top, thighs, arms or chin, too.

What Can a Mommy Makeover do for Your Belly?
Mommy Makeovers usually include a Tummy Tuck. The tummy tuck cinches the abdominal wall muscles, removes disproportionate fat and tightens loose skin, reversing much of what pregnancy does to the belly.
Sometimes less is needed, and there are several variations of the Abdominoplasty available including the “Floating Belly-Button” Tummy tuck and the Mini Tummy Tuck. What Can a Mommy Makeover Do For Your Breasts?
The other half of the Mommy Makeovers is Breast Enhancement. The most common enhancement is Breast Augmentation, which fills loose breast skin and enhances both volume and shape. For the loss of breast volume typical after childbirth, Breast Implants can fill the need.
When volume is not a problem, a Breast Lift may be what’s needed. The Breast Lift or Mastopexy, tightens the breast skin without changing the breast volume. Many different breast lifts exists, and the best Mastopexy for you will depend on how much lifting is needed.
Sometimes breasts increase in volume after pregnancy, and remain too large. Breast Reduction Surgery can help. A Breast Reduction, or Reduction Mammoplasty, will remove excess breast volume and tighten the skin, like a Breast Lift. The result is smaller, firmer and perkier breasts. This is especially helpful in cases where your breasts are heavy and causing pain in the neck, back or shoulders.
For mommies who had Breast Augmentation prior to pregnancy, your breast tissue changes with pregnancy, too. While the breast implant will retain it’s volume, sometimes the shape of the breast is impacted with the normal growth and involution of your breast tissue. When the shape is negatively impacted, Breast Implant Revision Surgery can help shape things up again.

So Liposuction or Tummy Tuck?
As much as I would like to perform minimally invasive surgery on everyone of my San Francisco Bay Area Plastic Surgery patients having abdominal rejuvenation, good results can only be obtained with good judgement. The first step toward a good result is defining what needs to be fixed. Once the problems are defined, the best procedure usually selects itself. This does not mean that compromises can’t be made, as long as it is understood that smaller procedures can only correct smaller problems.
Tummy Tuck vs. Liposuction Guide
Many specifics need to be reviewed to determine the best course of treatment for abdominal rejuvenation, and all of them are important. If I had to choose one, the most important factor is skin tone. If your skin tone is good (tight) and there are localized areas of fat in the abdominal wall or sides, then the indicator needle on the plastic surgery procedure meter points more toward Liposuction. On the other hand, if your skin is loose or redundant (folding over), especially if the underlying muscles are lax (commonly seen after weight loss or pregnancy), then the needle moves to Tummy Tuck.

Tummy Tucks Do What Diet and Exercise Cannot

Mommy Makeovers exist because babies are hard on their mommy’s “B’s”: Breasts and Bellies. The physiological changes that are necessary for a healthy child are not always beneficial for a healthy mommy’s body.
How to Repair the Aftermath of Pregnancy
The television video segment below is from this month’s Body Beautiful. I discuss Mommy Makeovers, the alternative to living with the results of pregnancy for both the Breast and Belly, from the perspective of a Board Certified Plastic Surgeon who has taken care of many mommies.
For the Breasts, options include: Breast Augmentation, Breast Lifts, Breast Reduction or a combination of these. Reduction of hypertrophied (overly enlarged) nipples and areolae is also possible. For Bellies: Liposuction, Mini Tummy Tucks and most commonly, Full Tummy Tucks are available. The video provides an introduction to many of the options available, including fat grafting to the buttocks (Brazilian Butt Lift), but cannot substitute for an in depth consultation with an experienced and Board Certified Plastic Surgeon.

Breast Enlargement for lactation is necessary, but when breasts shrink after breast feeding ends, the result can be empty, saggy breasts. Recent evidence points to pregnancy itself as the main culprit. Breast feeding, on the other hand, may instead offer benefits to both breast shape and baby wellness.
Why Breasts Sag With Pregnancy
Breasts are attached to the muscles behind them by long thin bands called Cooper’s Ligaments. When the breasts engorge, these ligaments elongate and cannot support the breasts as well. Immediately after breast feeding stops, the breast lose volume, which leads to further drooping. Some improvement occurs during the first six months after the cessation of lactation, so it is best to give the breasts a chance to recover, before planning your breast intervention.
There are many options for Breast Enhancement. The most common is Breast Augmentation, which restores lost volume and can help deflated breasts. When the breast enlargement that comes with pregnancy does not resolve, and the breast remain too large, Breast Reduction may provide relief from the excess weight and size of the breasts. If the size is good, but the breasts have dropped too low, a Breast Lift may be all that is needed.
What happens to the belly is a little simpler to understand. As the baby gets bigger, the belly stretches. This includes the skin and the muscle of the abdominal wall. Furthermore, the rectus muscles, which are a pair of muscles running down the middle of the belly, can separate and drift away from the midline causing abdominal weakness and diastasis.
What is Diastasis?
Diastasis is a separation of the rectus muscles. It is not a hernia and is usually not symptomatic or dangerous. A simple test for diastasis is to lay on your back, raise your shoulders off the bed/floor, and look at the center of your belly. Normally, when the shoulders rise the abdominal muscles tighten, and the abdomen flattens. With diastasis the mid-line, especially the area just above the belly button, will bulge upwards as the muscles tighten.
The bulge occurs because the muscles are no longer next to each other. As they are pulled apart with pregnancy, or weight gain, the middle of the abdominal wall is left unsupported. When the abdominal muscles tighten, the guts are squeezed and look for more room. The soft midline gives and provides additional space.
Repairing Diastasis is part of a Tummy Tuck, and one of the reasons that Abdominoplasty works better than diet and exercise for many mommies.
The most common abdominal component of the Mommy Makeover is the Tummy Tuck or Abdominoplasty. This provides repair of the muscle, tightening of the skin and removal of excess lower abdominal fat. If the problem is residual fat, and the skin is tight, Liposuction, alone may be all that is needed. When there is mild skin laxity isolated to the lower abdomen, a Mini Tummy Tuck could be a good compromise.
The Mommy Makeover has Lots of Options
