The first time I heard the term Liposculpture my first reaction was: good flamboyant marketing term. However, the more I thought about what Liposculpture implies, the more appropriate the term seems. The procedure for removing fat by vacuuming away localized collections is more descriptively termed Liposuction or suction assisted lipectomy.

Direct Lipectomy

Lipectomy is the generic term for the removal of fat (lipo) not the lip. Lipectomy has a long “i” like liposuction. Direct lipectomy is what is done with Tummy Tucks (Abdominoplasty). During a Tummy Tuck excess fat is removed with any loose skin from the lower abdomen by direct excision. Liposuction can be used to decrease fullness in the hips and around back, and this can lead to a shorter scar in cases where the skin tone is good. Liposuction is not recommended on the tummy during a Tummy Tuck, as this can lead to complications. Direct lipectomy, on the other hand, is performed under direct vision and can safely remove the deep fat during a Abdominoplasty.

Suction Assisted Lipectomy (SAL) aka Liposuction

Liposuction also removes fat. A thin cannula, 2 to 4 millimeters in diameter, is used to vacuum away unwanted fat. The advantages of liposuction over direct lipectomy include smaller incisions and the ability to place the incisions away from the areas of disproportionate fat. This allows me to place the incisions in “hidden” areas, while removing the fat from areas that may be exposed.

The term liposuction is descriptive of the method, and it does allow for the permanent removal of the fat cells from selected areas. Unlike weight loss, your metabolism and genetic programming don’t pick the areas where the fat leaves, you do.

Liposculpture

Liposculpture is not a method of liposuction. It is another term used to describe the process Liposuction. By exchanging the suction for sculpture, the emphasis is placed more on the artistry than the process. The engineer in me likes Liposuction, because it is a precise description of the mechanism, but Liposculpture appeals to my artistic side and emphasizes the versatility of technique.

According to Wikipedia:

• Sculpture is three-dimensional artwork created by shaping or combining hard materials – typically stone such as marble – or metal, glass, or wood. Softer (“plastic”) materials can also be used, such as clay, textiles, plastics, polymers and softer metals. The term has been extended to works including sound, text and light.

Fat can be added to the softer (“plastic”) materials. In the hands of a Board Certified Plastic Surgeon, fat too can be contoured to more visually pleasing form. Disporportionate bulges can be reduced, and in some cases, indentations filled by Fat Grafting. The ability to reestablish normal proportions to the body is the real power of Liposculpture. Fat is a living medium, so the results are not written in stone, but there is no better way to remove stubborn deposits of fat when the genetic plan won’t cooperate.

Weight Loss

Weight loss and Lipo are complementary processes. While there is overlap, neither can substitute for the other. Weight loss by definition is the reduction of weight. When we burn more energy than we consume, we lose weight. Our bodies are extremely efficient, and this efficiency can cause health problems. Today, the number of calories that would have taken an entire day to hunt or gather can now be obtained in just a few minutes, a few feet away. Whether it’s a short walk to the kitchen or a ride to the drive-through, calories can be found quickly and with a minimum of energy spent.

When we lose weight, it leaves from whatever area is genetically next in line. We cannot diet or exercise a specific body zone away. Sit-ups do not target the fat of the abdomen. Sit-up strengthen abdominal muscles, and burn calories, but those calories come from wherever your genetics say they should come from. You can’t choose.

Liposuction is not the solution to overconsumption, but it can be a great way to get a disproportionate body back into shape. Unlike weight loss, Liposuction does allow you to selectively remove areas of disproportionate fat. Liposuction does not cause you to burn more or consume fewer calories, however. Your weight is set by your metabolism, not by liposuction.

Liposuction – Like a Fad Diet with a Big But…

Removing fat with Liposuction is like removing fat with a crash diet. Unless other changes are made, when the diet is over, the weight returns. However, there is a big difference. Remember the part about getting to choose where the fat is removed? Unlike a fad diet, your genetics don’t choose where the fat leaves. Liposuction allows disproportionate areas of fat to be removed preferentially. This means that weight gain after Liposuction tends to be more proportionate.

Your Weight After Liposuction

The best candidates for Liposuction are near their ideal weight. One reason for this, is that the upper limit of weight removed with liposuction is around ten pounds. If someone is 100 pounds overweight, removing ten pounds is not likely to make a dramatic visual impact.

If you are near your ideal weight, you are likely to stay near your ideal weight. Fewer areas are likely to be disproportionate, and removing five pounds from a localized area can be a big change (4.4 pounds of fat will fill a two liter beverage bottle). If the weight comes back, usually it does not all come back in the same place, so the results will last.

Combining Weight Loss With Liposculpture

The combination of weight loss and Liposculpture is unbeatable. For some, weight loss alone will give a well proportioned body, but most of us have specific areas that collect the fat first. Out of proportion fat is the Achille’s heel of weight loss; however, the removal of disproportionate fat is the forte of Liposculpture. Liposuction can serve as a positive kick start for further weight loss. We naturally want to protect our investment, and once we can see positive results, we are motivated to make them even better.

Who Gets Tummy Tucks (Abdominoplasty)?

Many of my tummy tuck patients have had children. Their abdomens have stretched due to pregnancy, especially with multiple pregnancies. The baby, like weight gain, stretches the abdominal wall causing both muscle and skin laxity. This can occur even when physically fit. Abdominal wall stretching is compounded by even mild weight gain, as the fat seems to be stored preferentially in the lower abdomen.

Why is the Abdomen More Full at the Bottom?

Extra skin tends to fold over at the belly-button and at the belt line. This happens because most the attachments between the skin and abdominal muscles are loose, allowing gravity to slide the skin down over the muscles when we stand. Dense attachments between the skin and the muscle at the belly button and along the belt line keep the skin from sliding. Redundant skin above these areas accumulates and may fold over itself. The lower abdominal skin folding is accentuated by lower abdominal scars, like the scar from a C-section. The C-section scar firmly glues the skin to the underlying muscle, and gravity pulls any loose skin over the scar enhancing the pooch.

Another reason the abdomen is more full at the bottom has to do with the abdominal wall muscles. Pregnancy and weight gain increase the volume inside the abdominal cavity. This stretches not only the skin but also the tough fascial layers that support the abdominal muscles. When the fascia layers are stretched, the guts can push forward, causing a rounder belly. Tightening the muscles voluntarily can help, but when overstretched, it may not be possible to suck the guts in completely.

Fat retained inside the abdominal cavity can also cause a firm round belly that cannot be sucked in. Fat in this interior location cannot be removed with a tummy tuck or liposuction, and must be lost through diet and exercise. This is part of the reason it is best to be close to your goal weight before having your tummy tuck.

  Tummy Tuck: The Tummy Tuck, or Abdominoplasty, was created to tighten the abdominal wall muscles and to remove excess skin. A tummy tuck also removes excess fat more completely than liposuction. This three pronged approach addresses excess skin, excess fat and lax abdominal muscles. It is the most effective way to rejuvenate the shape of the abdomen. Abdominoplasty smoothes the skin, removes the fat and tightens the abdominal muscles.

  Liposuction: If the skin tone and muscle wall are already in good shape, Liposuction alone may be sufficient. However, if liposuction is performed with loose skin or muscles, the results will not be optimal. The muscle is not tightened with liposuction, and the skin may lose support from the fat removal and become more irregular.

  Mini-Tummy Tuck: The Mini Tummy Tuck, or Mini Abdominoplasty, can help when the muscle and skin laxity is isolated to below the belly button. It does everything a tummy tuck can do below the belly button, and can be combined with liposuction to address disproportionate fat above the belly button or on the sides. For laxity above the belly button, a tummy tuck is still the best choice.

Liposuction, specifically Tumescent Liposuction, remains the gold-standard in efficient, safe and predictable fat removal. Mesotherapy Banned

France recently banned one of the largest “alternative” fat removal treatments because it posses a significant risk to the patient’s health. The law was far reaching, and included ALL non-surgical alternatives to liposuction. The French court found that the inclusion of some already approved methods was inconsistent with the spirit of the law, and consequently has issued an injunction pending narrowing of the law’s scope.

France Keeps Liposuction - Bans the Rest.

France Bans Mesotherapy as a “Serious Health Risk”

France, the birthplace of Mesotherapy, has banned the practice. Fifty years after inventing Mesotherapy, France has now made it illegal. The French government has made Mesotherapy, Carboxytherapy and Laser Fat Reduction treatments illegal because they pose a “serious danger to human health.” In fact, all non-invasive of fat treatments (ultrasound, lasers, infrareds, radiofrequency) have been banned.

Originally published on April 12, 2011, and effective June 13, 2011, the Ministry of Work, Employment and Health, Decree N° 2011-382, prohibits mesotherapy and most other forms of aesthetic lipolysis (fat destruction). This ban includes many of the methods of fat removal currently marketed in the United States as safe, cheap alternatives to liposuction.

Liposuction – Still Safe and Effective

It is important to note that the ban does not include Liposuction. Liposuction remains the most predictable and most effective means of liposculpture. Unlike weight loss, Liposuction can selectively remove specific deposits of fat, and works best if you are maintaining a weight near your ideal.

Mesotherapy – A Serious Danger to Your Health

The notice from the French High Authority of Health prohibits specific techniques of aesthetic lipolysis (destruction of body fat cells) presenting a serious danger. It also bans techniques using appliances for external application (ultrasound, lasers, infrareds, radiofrequency) presenting a suspicion of a serious danger for human health. The full decree translated to English is printed below, with a link to the original French.

Decree N° 2011-382 Reads as Follows

• The Prime Minister, after report of the Minister of Work, Employment and Health, after the code of Public Health, especially of articles N° L.1151-3 and L.1152-2, after the advise of the High Authority of Health of December 17th 2010.
• Decrees:

  • Article 1 – The following techniques of aesthetic lipolysis are prohibited because of their serious danger for human health:
    • lipolysis using injections of hypo-osmolar solutions
    • lipolysis using injections of lipolytic solutions (phosphatidylcholine or sodiumdesoxycholate)
    • lipolysis using injections of mesotherapeutic mixture
    • lipolysis using Carboxy therapy(Carbon Dioxie or CO2)
    • lipolysis using transcutaneus Laser, without suction
  • Article 2 – Techniques of aesthetic lipolysis using appliances for external application are prohibited because of their suspicion of a serious danger for human health.
  • Article 3- The Minister of Work, Employment and Health is in charge of the execution of this decree, which will be published in the Journal Officiel de la Republique Francaise (<- click to read in the original in French).

American Plastic Surgery Societies Agree

The decree is in agreement with the advice given to its members by the American Society for Aesthetic Plastic Surgery (ASAPS) and the American Society of Plastic Surgeons (ASPS) task forces on mesotherapy. Both plastic surgery societies do not recommend their members use such methods as they remain unproven. The statement can be viewed on the ASPS website under Technology Briefs – Lipodissolve and Mesotherapy (Injection Lipolysis). Some of the devices included in the ban have previously been CE approved, so that will need to be sorted out by the courts.

How to Get Rid of Excess Fat

If you are near your ideal weight, and have stubborn disproportionate areas of fat that will not go away, liposuction can safely and effectively reshape your body. If you are significantly overweight, the safest method of weight loss remains a proper diet and regular exercise. Weight is likely to return after liposuction if not combined changes to your lifestyle to maintain the lower weight. If significantly overweight, liposuction is about as effective as a crash diet — once off the diet, the weight returns. If you are mobidly obese, bariatric surgery may be a viable option.
