The most subjective detail of Breast Augmentation is size. There is no mathematical formula or Breast Augmentation chart which allows you to calculate what size Breast Implant will be best for you.
Breast Implant Trial Sizes
Looking at before and after pictures online can help, especially if you are looking at women who started where you are starting, and who obtained a result you like. Your plastic surgeon may recommend increasing or decreasing your breast implant volume depending on your height and frame. The key is maintaining aesthetic proportions.
I’ve tried to review some of the key considerations here on the San Francisco Plastic Surgery Blog. Clicking on Breast Implant Options under the Breast Augmentation heading under Categories in the column to the left of this post, will link you with some helpful articles. Even so, there is no substitute for an in-person consultation with a Board Certified Plastic Surgeon.
High Strength Silicone Breast Implant Video
The following video is part 4 of 4 from a recent television appearance by me, Dr. Joseph Mele. During the show, I review the new gummy-bear, high strength, highly cohesive silicone gel breast implants that have been recently FDA approved. I also review Breast Augmentation in general, and answer some frequently asked question by viewers.

The video features frequently asked questions about Breast Augmentation including:
When is a Breast Lift needed?
When to consider Breast Augmentation with a Breast Lift?

Is a Breast Lift better for changing the shape of the breast?
Almost one third of all Breast Implant Revision Surgery is performed to replace a deflated breast implant. But what if breast implants couldn’t leak?
Saline Breast Implant Deflation
Saline Breast Implants can lose 100% of their volume through a small pinhole. Saline is essentially water, so a small rent is all that is needed for complete deflation. The water in a saline implant has a small amount of salt added so that is matches the natural saltiness of the body. Saline is quickly absorbed, just like when we drink water. There is no reaction to it, but the broken breast implant needs to be replaced.
Silicone Breast Implant Deflation
When a Silicone Breast Implant leaks, it may not be obvious. Unlike saline breast implants, the silicone gel that fills silicone breast implants is not absorbed by the body. The gel inside a silicone breast implant is cohesive, and sticks together, resisting flow. For traditional silicone breast implants, the FDA recommends an MRI at three years, and then every two years to be certain a “silent leak” has not occurred. When a leak is detected in a silicone gel breast implant, the breast implant and its contents need to be removed, and the breast implant needs to be replaced.
Deflation-Resistant Breast Implants
Breast Augmentation with the newest generation of breast implants, should significantly decrease the need for Breast Augmentation Revision Surgery. The Gummy-Bear Breast Implants are made with a high-strength silicone gel that behaves like a solid. Even with a large hole in the breast implant’s shell, the filling will not flow out.

This is part three of four of a discussion reviewing breast augmentation including the new gummy bear breast implants, made with high strength silicone gel. You can watch it here, on the San Francisco Plastic Surgery Blog.

Smooth Breast Implants vs. Textured Breast Implants
For years, board certified plastic surgeons have wanted a breast implant that does not leak. The high strength silicone breast implant may just be the answer. With the introduction of these “gummy bear” breast implants, Breast Augmentation is about to change.
High Strength Silicone Breast Implants
The high strength silicone filling in gummy bear breast implants is a soft solid. Even when the breast implant shell is cut in half, the silicone gel will not flow like water. The highly cohesive silicone gel sticks together, more like Jell-O than water. The less liquid like filler will reduce gel migration in the event that the shell is injured, leading to a more predictable Breast Augmentation.

Another advantage of the high strength silicone gel filler inside these new, next generation “gummy bear” breast implants is the ability to give the breast implants shape. In situations where there is very little breast tissue, especially after a mastectomy, a shaped implant can provide superior cosmetic results.
Teardrop Shaped Breast Implants
The more abrupt transition from chest wall to breast implant is most commonly seen when a round breast implant is placed in front of the pectoris muscle in a subglandular pocket. The “fake-look” can often be smoothed by moving the breast implant to a submuscular position. In cases when the muscle does not provide enough tapering of the upper pole of the breast, it can be smoothed further by using a breast implant that has a more gradual shape at the top. Teardrop shaped implants have been available in both saline and silicone filled implants in the past, but these are the first implants to combine a stable form implant with fewer leakage problems.
